Euglycemic Diabetes Keto Acidosis A rare side effect of SGLT Two inhibitors

Abstract :Euglycemic diabetic ketoacidosis (EuDKA) is an uncommon but increasingly recognised complication of diabetes, particularly associated with the growing use of sodium-glucose co-transporter 2 (SGLT 2) inhibitors. Unlike classic diabetic ketoacidosis (DKA), EuDKA presents with normal or only mildly elevated blood glucose levels, making timely diagnosis challenging. We report the case of a 70-year old woman with type 2 diabetes mellitus who was hospitalized for percutaneous transluminal coronary angioplasty (PTCA) and developed dapagliflozin-associated euglycemic diabetic ketoacidosis in the postoperative period, despite discontinuation of the drug prior to the procedure. This case underscores the importance of maintaining a high index of suspicion for EuDKA in patients previously on SGLT 2 inhibitors, especially in the perioperative setting. Early recognition and prompt management are crucial to prevent serious complications and improve clinical outcomes.
