Suspected New-Onset Hypersensitivity to Ceftriaxone (Monocef) in a Patient Recently Diagnosed with HIV During Hospitalisation for Acute Febrile Illness

Abstract :Adverse drug reactions, particularly allergic responses, pose significant challenges in managing immunocompromised patients with complex medication regimens. This is a report of a patient who developed an acute allergic reaction to Ceftriaxone (Monocef) that developed acutely in a patient with recently diagnosed HIV infection despite no prior history of Cephalosporin allergy receiving multiple concomitant medications for a febrile illness. On receiving Ceftriaxone, the patient developed cutaneous manifestations of an allergic reaction generalised maculopapular rash along with erythema and redness and generalised itching over the body and periorbital oedema. Clinical course was adapted successfully with the change in antibiotic and addition of corticosteroids. Initial investigations revealed microcytic hypochromic anaemia, hypoalbuminaemia with hyperglobulinaemia, and hyponatraemia. Other biochemical parameters were largely within normal limits. Urine microscopy showed few pus cells and Gram-positive bacilli, with culture reporting urethral contaminants. Following the identification of the Ceftriaxone allergy, it was discontinued, and intravenous Penicillin was initiated. The patient was also prescribed oral corticosteroids for 3 days to manage the allergic response. Antiretroviral therapy and other supportive treatments were continued
